New Patient Registration

To register at our Practice, please attend the surgery to collect the relevant forms.  We ask for two forms of identification, photo ID (for example a driving licence or passport) and a proof of address, (for example utility bill or bank statement).

All new patients registering with the practice are invited to attend for a health check with our healthcare assistant. You will be asked to fill in a questionnaire so that we have details about your medical history. Your blood pressure, weight and urine will be checked and you will have an opportunity to find out more about the practice and the services we offer.

When you attend for your health check please bring your medication list or medication with you.
